INSTRUCTIONS FOR BENEVOLENCE FORMS
Members:
Any member wishing to apply for help from Benevolence should fill out a Special Needs Request form. Please be sure to fill out the form completely and to include a copy of the bill you would like to be considered for payment. You are urged not to wait until the last minute to turn in your form so that there is adequate time to process your request. You will be notified first once the committee has made its decision, please do not contact the church office.
Non-members:
Non-members will fill out a form depending on their area of residence. Franklin County residents should fill out a Frankfort ROSM form. Residents of other counties should fill out a Special Needs Request form. Please be sure to fill out the form completely and to include a copy of the bill you would like to be considered for payment.
