To recieve a reimbursement from the church, please click here, download the form, fill it out and return it to the church office. Place the form in Raymond Hunt's mailbox, located on top of the file cabinets to the left of the secretary's work area. Also, be sure to include a copy of your purchase receipt.
If you would like to submit your form electronically, fill out the form and attach it to an email to Raymond Hunt at raykahunt@fewpb.net. Be sure to attach a copy of your purchase receipt.
